- Holder Event Dates (if any)
- Please list the reason why an Additional Insured clause is needed
Loss Payee
- Name of the Board of Education
- Name of Certificate Holder
- Vehicle information, VIN # and value
- Equipment description, lease # (if available), and value
- Does TIV need to be increased to reflect change?
EDA
- Name of Board of Education Information specific to the project(s), such as project number & start date
- Estimated Cost of the Project(s)
- Copy of EDA contract as it pertains to the insurance requirements
- Certificates of Insurance from all parties working in the construction naming the district as additional insured with respects to construction
